About the Role
As a Community Nurse within our innovative Virtual Care Team, you’ll combine your clinical expertise with virtual care technology to provide responsive, evidence-based and patient-centred care within the community.
You’ll work closely with patients, families and a multidisciplinary team to:

This is an opportunity to use your nursing skills in a dynamic and evolving model of care, where your clinical expertise, compassion and commitment can have a genuine impact on people's lives.
PLEASE NOTE: While this role utilises virtual care technology, it is not a remote or work-from-home position. The successful applicant will be site-based, reporting to a designated location within the Central Coast local health district.
You will work collaboratively with patients, families, and a multidisciplinary team to assess needs, develop care plans, and implement strategies that promote recovery, independence, and overall wellbeing.
